The IU Laser Safety Program ensures regulatory compliance and safe operation of Class 3B and 4 lasers. The Program requirements are derived from the American National Standard for the Safe Use of Lasers (ANSI Z136.1 - 2014). It applies to all laser users who work with Class 3B or 4 lasers at the Indiana University facilities.
All lasers used at IU must be registered with IUEHS using the LS-1 form found in the Quick Links on this page.
Prior to use of a Class 3B or 4 laser at IU, all users must:
- Read the Laser Safety Training Guide
- Complete the online Laser Safety Training provided by IUEHS
- Receive specific training for each laser from the Principal Investigator, including standard operating procedures, safety protocols, and proper personal protective equipment.
All training must be documented on the LS-2 form in the Quick Links.
